Below are the State Fall Games Paperwork Packets that each agency needs to complete for their 2026 Fall Games Registration. If you have questions you may contact your Program Manager.
Paperwork for EVERY agency:
SOGA has created a downloadable registration packet that includes all required forms and event entry forms for agencies registering for Fall Games. Please see the link below to access the registration paperwork. A printable PDF version of the registration packet is available for agencies to complete by hand. Once completed, the packet must be either mailed or scanned and emailed to SOGA. Lastly, there is an additional packet containing SOGA policies, rules and sports specific information that agencies need to download and review.
