Below are the State Fall Games Paperwork Packets that each agency needs to complete for their 2021 Fall Games Registration. If you have questions you may contact your Program Manager.
Paperwork for EVERY agency:
SOGA has created a downloadable registration packet, that includes all of the required forms and event entry forms for Agencies to register for Fall Games. One of the downloadable registration packets is in a electronic, fillable format in Microsoft Word. Agencies can input their registration information via the computer easily and neatly. They can then save their packet and email it and/or print it and mail it to SOGA. The second downloadable registration packets is a PDF, that agencies can print and hand-write their information. They can then scan and email it or mail it to SOGA. Lastly, there is an additional packet containing SOGA policies, rules and sports specific information that agencies need to download and review.
Completed paperwork is due to the SOGA Valdosta Office by 4:15 pm on August 20, 2021. The mailing address is 3998 Inner Perimeter Road, Suite A, Valdosta, GA 31602. Please put it to the attention of David Crawford. Agencies will incur a $250 Late Paperwork Fee for paperwork received after August 20, 2021! Paperwork will not be accepted by SOGA after Wednesday, August 25, 2021!!
